GP Adolescent Referrals (14-18)

The community based adolescent service is based at the Dean Clinic Lucan and the Willow Grove in-patient service is based on campus at St. Patrick’s University Hospital. The Willow Grove Adolescent service is completely separate and distinct from Adult units at the Hospital.

If your client is aged 14-18, and you wish to refer them to our Adolescent Dean Clinic, you will need to complete an Adolescent Dean Clinic Referral form.

If you feel that your client needs to come directly into Hospital, you will need to complete a Willow Grove Inpatient Referral form. 

When the referral form is received it will be assessed by the team and a decision will be made on whether the referral is appropriate. If this is the case a Dean Clinic appointment or an assessment at Willow Grove will be arranged.
